Community Care Connections (CCC) is a disability support organization serving adults and children with developmental delays and disabilities across Southwestern Pennsylvania. For more than 50 years, we have been committed to empowering individuals with disabilities to live safely and independently in their homes, schools, neighborhoods, and communities.

Founded in 1972 as United Cerebral Palsy of Butler, Beaver, and Lawrence Counties by parents of children with disabilities, CCC originally focused on providing day programming for children and adults. In 2003, we became Community Care Connections, expanding our mission and services to meet the evolving needs of the community. Today, we serve more than 700 individuals in six western Pennsylvania counties.

Our supports and services include:

  • Children’s Services including Early Intervention for children birth to 36 months and their families, delivered by licensed therapists and, Camp Apple, a free summer camp and year-round program for children ages 3–21, designed to foster inclusion by welcoming both children with and without disabilities.
  • Adult Services which include attendant care service coordination, homemaker chore, employment supports – including benefits counseling, community participation supports – including center-based supports, behavior supports, supported living and 12 licensed community homes.

CCC employs 150 dedicated professionals providing residential and day supports to adults with disabilities. Our team also includes early intervention therapists, seasonal camp staff, and highly qualified administrative personnel working across 14 locations and throughout the community.

At Community Care Connections, we believe in creating opportunities, breaking down barriers, and building connections that enrich lives.
