Community Care Connections’(CCC) Community Homes Service is designed to support adults with intellectual disabilities in a safe and welcoming, residential environment. This service helps individuals develop, maintain, and enhance daily living skills, while fostering independence and community involvement. CCC operates 12 community homes in the Butler area, each offering a comfortable setting where individuals can thrive. All homes are licensed and monitored by Pennsylvania’s Office of Developmental Program (ODP) Our services include:
- Skill Development: Support with self-care, communication, socialization, and adaptive skills.
- Community Engagement: Opportunities to participate in meaningful activities and build lasting connections.
- Health and Safety: Continuous, individualized support and advocacy to ensure their well-being and meet health related outcomes.
- Person-Centered Planning: The service is tailored to each individual’s goals through their Individual Support Plan.
Our dedicated Direct Support Professionals (DSPs), Program Specialists, and House Coordinators are highly trained and committed to providing compassionate, person-centered care. Licensed Behavioral and Employment Specialists are also on-staff and available to provide additional supports as needed or desired. The expertise and dedication to our mission of this incredible team ensures that every individual is receiving the supports they need to live their best life at home and in the community.
In 2025, CCC achieved the Select level of performance in ODP’s performance-based contracting model for residential services, one of only 15 providers in Pennsylvania to do so. Being a Select provider indicates that CCC met the standards required of all providers and met additional enhanced measures related to quality, access, workforce stability and clinical capacity. These measures are designed to ensure higher quality and continuity of care for individuals with intellectual disabilities sand autism.
