Supported Living are direct and indirect services provided to participants who live in a private home that is owned, leased or rented by the participant or provided for the participant’s use via a Special or Supplemental Needs trust and located in Pennsylvania. Supported Living services are provided to protect the health and welfare of participants by assisting them in the general areas of self-care, health maintenance, wellness activities, meal preparation, decision making, home management, managing personal resources, communication, mobility and transportation, relationship development and socialization, personal adjustment, participating in community functions and activities, and use of community resources. Through the provision of this service participants will be supported to live in their own home in the community and to acquire, maintain, or improve skills necessary to live more independently and be more productive and participatory in community life.

At Community Care Connections, Inc., we go beyond the basic support of the Supported Living service to make independent living a reality. From the very first steps toward independence to achieving personal goals, our proactive team is there every step of the way. We help participants find apartments that fit their needs and preferences, offer practice apartments to build confidence, and provide access to volunteer opportunities and community resources. Technology is a big part of what we do—whether it’s using Ring cameras for safety, PillDrill for medication reminders, or phone settings like voice-to-text to make life easier. Guided by our motto, “Empower, not enable,” we use tools like the Integrated Supports Star and Charting the Life Course to create personalized plans that foster independence, safety, and meaningful connections
