Community Care Connections offers two residential service options under Pennsylvania’s office of Developmental Programs (ODP) guidelines: Supported Living and Licensed Residential Habilitation. Supported Living provides individualized assistance and transportation to help people live in their own homes or apartments, promoting independence and community integration. Our Licensed Community Homes offer 24/7 support in our agency-owned homes licensed by the Department of Human Services. Services provided in these homes focuses on skill development, health, and safety while ensuring a person-centered approach. Both services are designed to help individuals acquire, maintain, and improve daily living skills so they can live as independently as possible and thrive in inclusive community settings.
