The Community Care Connections’ Community Supports Program offers a variety of services funded through the Department of Human Services via Medicaid’s Consolidated, PFDS and Community Living Waivers.
The Community Supports Program offers participants 2 ways to receive “community participation supports.” Both options provide safety and are suited to the needs of adults with intellectual and developmental disabilities and the needs of older adults.
The Golden Opportunities Center is located on Whitestown Road in Butler. The “GO Center” offers a center-based approach to community integration and opportunities to engage with peers. Alternately, participants may choose to receive services in a 100% community-based setting. These Community Participation Support programs are dually licensed by the Department of Human Services and the Department of Aging.
The Community Supports Program also provides “in-home and community supports” to adults with intellectual and developmental disabilities who live in a private/family home. This service is designed to assist with skill-building toward independent living. Services are delivered in the participant’s own home and community.
In 2019, the Community Supports Program began providing employment services including “job coaching,” “discovery” and “job development customized employment.” The employment services are based on the participant’s skills, interests and talents. Some employment services are funded by OVR, the Office of Vocational Rehabilitation, while others are funded through the Department of Human Services.
Regardless of what services one receives or where their services take place, the Community Supports Program offers flexible scheduling and person-centered planning to help each participant achieve an everyday life.
