Attendant Care Service Coordination Program

At Community Care Connections, our Attendant Care Service Coordination Program provides adults with disabilities, who wish to receive attendant care services, access to service coordination resources. Our program assists members in developing an Individual Service Plan, which outlines any personal and housekeeping services the individual may require for safe living in their homes and communities.

Your Attendant Care Service Coordinator will connect you with local providers who will directly administer the needed personal care and housekeeping services outlined in your Individual Service Plan. They will also serve as a liaison between you and your providers, ensuring that the proper frequency, duration, and quality of services, as stated in your Individual Service Plan, is being upheld. As your time in the program progresses, your Service Coordinator will perform reassessments to amend your Individual Service Plan to reflect any changes in health and/or living arrangements.

The Attendant Care Service Coordination Program is funded by the Long-Term Care Services Office within the Pennsylvania Department of Human Services. At Community Care Connections, we offer two program options: the OBRA Waiver and Act 150. Read on for explanations of these programs and to learn if you are eligible.

 

OBRA Waiver

The OBRA Waiver helps individuals with a developmental physical disability to remain living as independently as possible and thrive in their communities.

Eligibility: 

  • A resident of Pennsylvania
  • Aged 18-59
  • Meet the determined finance requirements as enforced by your local County Assistance Office
  • Have a severe developmental physical disability that results in substantial functional limitations in three or more of the following:
  • Self-Care
  • Communication
  • Learning
  • Self-Direction
  • Capacity for Independent Living
  • Mobility

Act 150

The Act 150 program is for adults with physical disabilities who require minimal assistance to continue living independently.

Eligibility: 

  • A resident of Pennsylvania
  • Aged 18-59
  • Meet the level of care needs for a Skilled Nursing Facility
  • Capable of managing:
    • Attendant care worker(s) (i.e., hiring, firing, and supervising)
    • All personal financial affairs
    • All personal legal affairs
  • Capable of directing personal care
  • Have a severe, medically determinable physical impairment expected to last twelve months or longer.
  • Determined financially ineligible for Medicaid (MA)
  • Assessed a minimal co-payment based on income levels that will not exceed the total costs of services.

Possible Services: 

  • Personal Assistance
    • Bathing, toileting, dressing, etc.
  • Personal Emergency Response Systems (PERS)
  • Service Coordination
  • Financial Management Services

Program Cost

Depending on the chosen program, services through the Community Care Connections Attendant Care Service Coordination Program are provided free-of-charge to income-eligible adults or on a sliding fee scale. Payment requirements will be tailored to your personal financial situation.
