Act 150 is a long-term care program for individuals with physical disabilities who need minimal assistance to continue living as independently as possible in their homes and communities. The goal of Act 150 is to provide services for maintaining an improved quality of life that revolves around physical actions that may be difficult or not feasible for individuals with certain physical disabilities. At Community Care Connections, we offer access to Act 150 services through our Attendant Care Services Coordination Program. This program is for individuals whose income exceeds the limit for Medicaid but who still need financial support. The program is funded by the Pennsylvania Department of Human Services to eligible individuals. Services through the Community Care Connections Attendant Care Service Coordination Program are provided free of charge to income-eligible adults or on a sliding fee scale. Payment requirements are tailored to your financial situation.
