How Does Act 150 Benefit You?

What is Act 150?

Act 150 is a long-term care program for individuals with physical disabilities who need minimal assistance to continue living as independently as possible in their homes and communities. The goal of Act 150 is to provide services for maintaining an improved quality of life that revolves around physical actions that may be difficult or not feasible for individuals with certain physical disabilities. At Community Care Connections, we offer access to Act 150 services through our Attendant Care Services Coordination Program. Services through Act 150 look to achieve at least one of the following objectives:

• To live in the least restrictive environment as independently as possible
• To remain in their own home and prevent unnecessary institutionalization.
• To receive in-home personal care
• To receive Service Coordination/Case Management Services

Eligibility

• A resident of Pennsylvania
• Aged 18-59
• Meet the level of care needs for a Skilled Nursing Facility
• Capable of managing:

• • Attendant care worker(s) (i.e., hiring, firing, and supervising)

• • All personal financial affairs

• • All personal legal affairs

• Capable of directing personal care
• Have a severe, medically determinable physical impairment expected to last twelve months or longer.
• Determined financially ineligible for Medicaid (MA)
• Assessed a minimal co-payment based on income levels that will not exceed the total costs of services.

Possible Services

• Personal Assistance

• • Bathing, toileting, dressing, etc.

• Personal Emergency Response Systems (PERS)
• Service Coordination
• Financial Management Services

Program Cost

This program is for individuals whose income exceeds the limit for Medicaid but who still need financial support. The program is funded by the Pennsylvania Department of Human Services to eligible individuals. Services through the Community Care Connections Attendant Care Service Coordination Program are provided free of charge to income-eligible adults or on a sliding fee scale. Payment requirements are tailored to your financial situation.
