The Community Care Connections’ Community Home Program provides residential placement for adolescents and adults with intellectual disabilities.
CCC operates nine homes in the Butler area. Each site is home to three individuals. Each resident has his/her own bedroom which they may decorate according to their interests and preferences. Common areas of each house are clean and comfortably furnished, creating a warm family-like environment. Accessibility modifications including ramps, grab bars or tub lifts are installed at the homes to accommodate individuals with special health or mobility needs.
Specially trained staff members are always present to provide each person with the individual support and assistance they require. Each person has an individual service plan and may attend a day program or prevocational program. Evening and weekend activities are scheduled around the interests of each house member. Family visits are encouraged as are trips home for holidays or special occasions.
The Community Care Connections’ Community Home Program is licensed and funded by the Office of Developmental Programs.
